Use this form within 30 days of receiving your order. If Medigy sent the wrong item(s), attach proof and you pay nothing once verified. Otherwise exchanges are $25 unopened (includes your return label) or $45 opened; refunds apply to unopened items only, less a 30% restocking fee. Returned items are inspected on arrival.
